Occupational Therapist - Rehab & Discharge Planning
Snapcare
SnapCare in Cleveland, OH seeks an experienced Occupational Therapist (OT) to provide critical illness and rehabilitation services within an interdisciplinary care team.
You will assess patients, develop individualized treatment plans, supervise Certified Occupational Therapy Assistants and OT/COTA students, and complete timely evaluations and progress notes while ensuring safety, documentation, and effective discharge planning.
