RN Patient Flow Clinical Expediter- Capacity Command Center

Location: METROHEALTH MEDICAL CENTER, CLEVELAND, OH Biweekly Hours: 80.00 Shift: 8:30 a.m. - 7:00 p.m. Job Summary Facilitates hospital throughput by actively managing patient progression, identifying delays in care, and removing barriers to timely discharge. Focuses on optimizing movement throughout the care continuum—with a strong emphasis on examining discharge readiness, discharge barriers, and coordination with the Discharge Hospitality Center. Partners closely with Command Center leadership to align patient flow including actions supporting utilization review and system-wide capacity demands. Understands and supports the centralized Bed Management, admission, discharge, and throughput policies, procedures, and processes. Upholds the mission, vision, values, and customer service standards of The MetroHealth System. Qualifications Required: Current Registered Nurse License in the State of Ohio Bachelor’s degree in Nursing (BSN) Minimum three years of RN clinical experience in critical care or a combination of critical care and emergency care Knowledge to triage patients to the appropriate level of care when planning bed placement or facilitating care at admission Proven excellent customer service skills Ability to work independently and as a member of a multidisciplinary team Ability to communicate effectively with physicians, other health care professionals, patients, and families Ability to utilize Epic, paging system, and other necessary computer applications to perform role responsibilities Proficiency with computers and Microsoft applications Ability to interact effectively with a wide range of cultural, ethnic, racial, and socioeconomic backgrounds Preferred: Two years of experience with bed management and/or transfer center Experience with patient flow at MetroHealth Medical Center Nursing supervisory experience Physical Demands May need to move around intermittently during the day, including sitting, standing, stooping, bending, and ambulating; may need to remain still for extended periods.
