Job Summary Position is responsible for providing comprehensive nursing services to clients and their families, including assessment, education, and coordination of care with a multidisciplinary team in a primary care and family practice setting. Intra‑departmental Relationships Department Officer: Chief Operations Officer Reports to: Nursing Director Supervises: None Primary Responsibilities Participate as an integral part of a multidisciplinary team for comprehensive health care of clients and families. Provide triage assessment, consultation, and referral for FCHC clients contacting via telephone, patient portal, or clinic. Consult with medical provider as needed to determine care level. Schedule same‑day appointments in conjunction with provider. Provide appropriate education and home‑care advice. Obtain information for clients requesting test results and medication refills. Document assessment, patient education, nursing interventions, and encounter notes in the EMR. Sends refill requests to the appropriate provider and completes prior authorization when needed. Perform routine direct patient care such as administering injections, dressing changes, vital sign assessment, and other RN duties. Develop individualized care plans setting mutual health goals with patients/families for acute, complex, and chronic conditions. Provide education about disease processes (e.g., diabetes, asthma, COPD) emphasizing prevention and promotion. Consult with multidisciplinary team members (Community Health, Nutrition, Behavioral Health, Pharmacy, etc.) to tailor interventions. Coordinate care using a care‑management approach: risk assessment, referral, follow‑up, crisis intervention, documentation, and care‑plan modification. Provide medication education and support for health‑condition management. Conduct post‑hospital discharge transitions of care, oversee follow‑up scheduling, and document medication reconciliation. Communicate and facilitate direct and indirect patient‑care activities, including coordination with Pharmacy, Referrals, DME/Home Health/PT/OT, Insurance, Health Care Organizations, Schools, Community Organizations, and other partners. Participate in huddles and pre‑visit planning with provider and care team members. Work with providers to support team efforts around care coordination and model quality metrics (HRSA, PCHH, ACO, etc.). Attend learning sessions and collaborative calls with partners (Missouri Primary Care Association, Department of Health and Senior Services, etc.) as needed. Interface with DRVS and Cyber Access to monitor individual care plan effectiveness for Medicaid patients. Provide support and assistance to other RN team members to ensure consistent, quality patient care. Respond to medical emergencies in the office. Act as a resource for Medical Assistants and other personnel as needed. Participate in facilitating a learning environment for students. Collaborate with self‑directed management teams to improve the delivery of health services. Periodic Duties Participate in Health Center staff problem‑solving groups. Attend and participate in department meetings. Identify and present appropriate educational materials for different audiences. Present information to various audiences. Participate in data analysis and quality assurance. Perform other duties as assigned. Working Relationships Inside Center: All inclusive. Outside Center: Patients/Clients, students, vendors, community health agencies, hospitals, physician’s offices, Dept of Health & Social Services, etc. Skills and Abilities Excellent clinical skills. Intermediate proficiency with EMR, Outlook, and MS Office (Word, Teams). Excellent telephone and customer service skills. Excellent teamwork and collaboration abilities. Professional attitude, demeanor, and respectful communication. Qualifications Graduate of an accredited school of nursing; BSN preferred. Registered Nurse licensure in the State of Missouri in good standing. At least 2 years of nursing experience with patients having complex conditions requiring multidisciplinary care coordination. Experience in a primary care setting; OB experience preferred. Continuing Education Satisfactory maintenance of continuing education as required by the Missouri Nurses’ Association.
