Family Medicine Physician – Outpatient Primary Care

NHSC & NYS Loan Repayment Eligible | Rochester, NY
Anthony L. Jordan Health Corporation is seeking a Board Eligible or Board Certified Family Medicine Physician to join one of the nation's oldest Federally Qualified Health Centers serving the Rochester community for more than 120 years.

Requirements:

 

Responsibilities

• Provide comprehensive outpatient primary care to pediatric, adult, and geriatric patients. 
• Perform preventive care, acute care visits, chronic disease management, and health screenings. 
• Develop and maintain continuity of care for an assigned patient panel. 
• Collaborate with nurses, behavioral health providers, pharmacists, care managers, and other interdisciplinary team members. 
• Refer patients to specialty care and community resources as appropriate. 
• Utilize evidence-based medicine and participate in quality improvement initiatives. 
• Serve as a preceptor for residents and students as assigned. 
• Maintain accurate and timely documentation in the electronic health record. 
• Participate in departmental meetings and organizational initiatives.

Education and Experience Required: · Possession of a license to practice as a medical doctor in New York State. 
Licenses and Certifications: · Board eligibility or Board certification.· Must obtain credit of CME as required. · Must meet privileging and credentialing requirements for Physicians. · Infection Control certificate. · BLS through the American Heart Association.
