Our company prioritizes proactive measures for enhanced physical health and well-being, incorporating Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) into our comprehensive preventative care program.Responsibilities:Provide high-quality preventative care to residents in skilled nursing facilities through on-site visits and remote work from homeConduct assessments, develop treatment plans, and prescribe medications as neededCollaborate with healthcare teams to ensure comprehensive care and minimize hospitalizationsUtilize RPM, CCM, and CoCM to enhance patient outcomesMaintain accurate and timely documentation of patient care and outcomesParticipate in quality improvement initiatives to enhance patient care standardsRequirements:Active NP license (in the state of practice)1+ year of experience in Geriatric care and documentation reviewExcellent communication, collaboration, and teamwork skillsBenefits:Hybrid role with flexible scheduling (8 hours per week)Support and Training from our clinical directors, regional managers, and supervisorsGenerous Paid Time offCompetitive annual salary range of $20,000 - $22,000Join our team and experience the benefits of working with Carease Health - IL!
