We are currently seeking a caring and dependable Registered Nurse (RN) to join our Home Health team serving Portsmouth NH and surrounding communities. If you are passionate about patient-centered care and looking for a flexible and supportive work environment this opportunity is for you.

Responsibilities

• Provide one-on-one skilled nursing care to patients in their homes
• Assess patient conditions and develop individualized care plans
• Administer medications treatments and other nursing interventions as prescribed
• Monitor patient progress and communicate changes to physicians and healthcare team members
• Educate patients and family members regarding treatment plans medications and self-care techniques
• Maintain accurate and timely clinical documentation
• Collaborate with interdisciplinary healthcare professionals to support patient goals and quality outcomes
• Ensure compliance with all state federal and company regulations and standards

Qualifications

• Current Registered Nurse (RN) license in the State of New Hampshire
• Minimum of one (1) year of nursing experience within the last five years preferred
• Home health or home care experience preferred
• Current CPR Certification required
  (Monthly renewal classes available and paid for by Interim HealthCare)
• Strong communication and clinical assessment skills
• Compassionate dependable and patient-focused approach to care
