Home Infusion Clinical Pharmacist
Option Care
Option Care is seeking a Clinical Pharmacist to provide pharmacy services to patients in home care settings. The role involves assessing patient suitability, designing pharmacy care plans, and educating patients and caregivers on medication use. The ideal candidate must be a graduate from an accredited School of Pharmacy and hold a current pharmacy license. Experience in sterile IV admixture procedures is preferred.
