Salary: $136,406.40 $200,000 annually
Job Summary
The Population Health Clinical Pharmacy Specialist provides clinical pharmacy support to participating health plans and providers involved in value-based care arrangements. The role focuses on improving pharmacy-related quality measures, supporting medication-related care, reducing avoidable costs, and improving health outcomes for Medicare and commercial populations.
Key Responsibilities- Analyze reports and EMR data to identify opportunities to improve medication use, STAR measures, HEDIS measures, and pharmacy cost containment.
- Assess patients for barriers to care and provide medication therapy management and self-management education.
- Develop personalized health improvement plans and goals.
- Collaborate with providers and care teams on clinical quality initiatives.
- Support medication-related quality measures for value-based care agreements.
- Educate providers and health plan members regarding formulary changes.
- Identify opportunities to prevent avoidable hospital admissions and readmissions.
- Participate in committees, meetings, audits, and policy development.
- Serve as a pharmacy subject matter expert for clinical quality measures.
- Bachelor's degree in Pharmacy.
- Active pharmacist license.
- Board of Pharmacy Specialties certification.
- BPS certification in Pharmacotherapy, Ambulatory Care, Cardiology, Geriatrics, or a related specialty .
- One year of hospital pharmacist experience as a clinical pharmacist, or completion of an ASHP-accredited PGY-1 residency program.
- Strong written and verbal communication skills.
- Strong analytical and problem-solving abilities.
- Doctorate degree in Pharmacy.
- Board certification with clinical pharmacy experience.
- PharmD with advanced board certification.
- Advanced degree such as MPH or MBA.
- Two years of clinical pharmacy experience or completion of an ASHP-accredited PGY-2 ambulatory residency
