Minimum Requirements

• Associate's, Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program.
• Current Physical Therapy licensure in South Carolina required to start the assignment.
• Current CPR certification required.
• Current driver's license, vehicle insurance, and access to a dependable vehicle or reliable public transportation for patient visits.
• Experience providing home health physical therapy preferred, including patient evaluations, developing PT plans of care, progress documentation, and coordinating with interdisciplinary teams.
• Ability to perform patient evaluations, develop and implement treatment plans, and document outcomes per agency policy and state guidelines.

• Shift Name: Days.
• Shift Details: Days 0800-1600.
• Weekly Hours: 40 hours per week (5x8 schedule).
• Coverage Areas: Chesterfield County, Darlington County, and Marlboro County, SC.
• Mileage reimbursement provided between first and last patient visit.
