Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of seniors to identify their psychosocial needs, understanding their emotional and social contexts.

• Develop, implement, and monitor Individualized Family Service Plans (IFSPs) in collaboration with interdisciplinary team members.

• Provide regular support to families, equipping them with resources and strategies to enhance their loved ones' quality of life.

• Engage in case management to coordinate various services tailored to meet clients' specific needs, ensuring holistic treatment approaches.

• Adhere to all federal and state regulations concerning social work practices and client confidentiality.

Qualifications\n

Compensation and Benefits\n
