Position Summary

The Day Program Services Coordinator supports the delivery of high-quality, person-centered services by coordinating participant care, documentation, communication, and program support activities. This position serves as the primary liaison between participants, families, interdisciplinary teams, and program staff to ensure individualized services are coordinated, documented, and delivered in accordance with organizational standards and regulatory requirements.

Essential Duties and Responsibilities

Participant Services & Coordination

• Coordinate participant enrollment, attendance, schedules, and ongoing service needs.
• Promote participant independence while supporting individual goals, preferences, and quality of life.
• Monitor participant engagement and communicate concerns or changes to the appropriate team members.
• Provide direct participant support, including personal care assistance, when needed.

Care Planning & Documentation

• Coordinate and facilitate participant Care Plan meetings.
• Maintain complete and accurate participant documentation within the electronic health record.
• Ensure required assessments, service documentation, incident reporting, family notifications, and participant records are completed within established timelines.
• Maintain documentation in accordance with organizational policies and regulatory requirements.

Program Support

• Collaborate with instructors and program staff to ensure participant needs are reflected in daily programming.
• Assist with coordinating on-campus and community-based activities that promote meaningful engagement and skill development.
• Support implementation of individualized goals identified through care planning and functional assessments.

Communication & Collaboration

• Serve as a liaison between participants, families, guardians, instructors, and leadership.
• Foster effective communication to support continuity of care and positive participant outcomes.
• Assist in resolving participant concerns through collaborative problem-solving.

Quality & Compliance

• Support quality improvement initiatives through accurate documentation, record maintenance, and process improvement efforts.
• Perform other duties as assigned in support of departmental and organizational goals.

Qualifications

Required

• Bachelor's degree in Human Services, Social Work, Psychology, Special Education, Rehabilitation, or a related field.
• Minimum of two (2) years of experience working with individuals with intellectual and developmental disabilities or in a related human services setting.
• Valid Georgia Driver's License with an acceptable motor vehicle record.
• Ability to meet all pre-employment screening requirements, including criminal background, drug screening, and tuberculosis screening.

Preferred

• Experience with person-centered planning and interdisciplinary service coordination.
• Knowledge of Applied Behavior Analysis (ABA) principles and functional assessments.
• Experience with electronic health records and documentation systems.

Knowledge, Skills & Abilities

• Knowledge of person-centered service delivery and best practices for supporting individuals with intellectual and developmental disabilities.
• Strong organizational, communication, and interpersonal skills.
• Ability to coordinate multiple priorities while maintaining attention to detail.
• Ability to build collaborative relationships with participants, families, and interdisciplinary teams.
• Proficiency with Microsoft Office and electronic documentation systems.

Physical Requirements

• Ability to frequently sit, stand, walk, bend, kneel, crouch, reach, and lift up to 25 pounds independently and up to 75 pounds with assistance.
• Ability to safely transport and assist participants entering and exiting vehicles.
• Ability to obtain and maintain CPR and First Aid certification.
• Ability to meet the physical demands necessary to provide direct participant support in both campus and community settings.
