This contract opportunity focuses on field-based care, providing individualized physical therapy to patients within their homes and making a genuine difference in their daily lives.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Responsibilities:

• Conduct thorough initial and ongoing physical therapy evaluations, including OASIS assessments when applicable.
• Develop tailored rehabilitation plans targeting functionality, mobility, strength, balance, gait, and pain management.
• Deliver skilled therapeutic interventions such as exercise programs, gait training, transfer training, fall prevention, and neuromuscular re-education.
• Monitor patient progress; update treatment plans based on assessments and communicate with the care team and physicians as indicated.
• Provide clear education for patients and caregivers on exercises, the safe use of assistive devices, and home-based rehabilitation strategies.
• Complete timely and accurate documentation using the Home Care Home Base (HCHB) electronic record system.
• Collaborate closely with nurses, occupational therapists, speech therapists, social workers, and assistants for coordinated home care.
• Attend team conferences, professional meetings, and mandatory continuing education sessions.
• Uphold high standards aligned with Medicare home health guidelines and agency policy.

Qualifications:

• Active Physical Therapist license (PT) in good standing for the state of practice.
• At least one year of recent home health physical therapy experience (field work preferred).
• Proficiency with HCHB and required experience with OASIS assessments.
• Strong clinical reasoning, assessment, and communication abilities.
• Reliable transportation, valid drivers license, and vehicle insurance.
• Comfortable working independently and managing a flexible, field-based schedule.

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
