HIV Care Navigator - Bilingual Case Manager

St. Johns Riverside Hospital

St. John's Riverside Hospital in Yonkers, NY, is seeking a Care Manager to provide patient-centered, strengths-based care management for individuals living with HIV. You will coordinate outreach, assessment, care planning, and implementation with the patient and care team to promote better health outcomes. Ideal candidates have a bachelor’s degree in human services, 3+ years in health-care case management (HIV/AIDS/substance use), bilingual English/Spanish, a valid driver's license, and strong #J-18808-Ljbffr St. Johns Riverside Hospital
