Responsibilities

• Assess residents’ PT needs; develop and implement individualized treatment plans
• Deliver PT interventions including therapeutic exercise, gait and balance training, and manual therapy as appropriate
• Reassess progress, adjust plans, and document outcomes in the patient record
• Educate patients and families on exercises, safety, and home programs; promote functional independence
• Collaborate with an interdisciplinary team to coordinate care and discharge planning
• Ensure patient safety, infection control, and regulatory compliance during therapy sessions

Requirements

• Active Physical Therapist license in California or eligible for CA licensure
• Experience in a skilled nursing facility or geriatric rehabilitation setting preferred
• Availability to commit to a 13-week travel assignment with 30 guaranteed hours per week
• Ability to start by 08/01/2026 and work the designated weekend-based schedule (Sun–Thu or Tue–Sat)
• Strong communication and teamwork skills; ability to work independently and as part of a team
