In this role, you will: Conduct comprehensive assessments and develop individualized service plans Provide ongoing participant-centered case management and support Coordinate housing stabilization efforts and connect families to critical resources Facilitate referrals and collaborate with community partners and service providers Help individuals and families navigate barriers related to housing, employment, benefits, healthcare, and other essential services This position offers the opportunity to make a meaningful impact while enjoying a predictable weekday schedule with no regular evening, weekend, or holiday requirements.
