Registered Nurse Care Manager

Team Based Care (TBC) Manager RN The Team Based Care (TBC) Manager RN independently uses the nursing process to care for a group of patients safely, therapeutically, and efficiently. This role conducts shared patient visits with providers as well as independent RN visits using standardized procedures, protocols and documented workflows. The TBC RN collaborates with other team members to coordinate and execute the plan of care and demonstrates initiative in the delivery of patient care in a Team Based Care Model. Actions reflect the Providence core values. Required Qualifications Graduate from an accredited school of Nursing. Upon hire: California Registered Nurse License. Minimum of 2 years Professional Nursing Experience. Preferred Qualifications Bachelor’s degree in Nursing. At least 1 year of experience in ambulatory care nursing. Additional compensation may be available for this role, such as shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.
