Licensed Practical Nurses (LPN's)

Concord Center for Nursing and Rehabilitation  is actively hiring LPN Team Members for our Skilled Nursing Facility in Brooklyn, NY.

Now Offering a $7,500 Sign-On Bonus, which is paid out in installments over the first year of employment.

Base rate is $33.54 per hour with an additional 10% shift differential for evenings and nights ($36.89 .)

DUTIES:

• Collecting required information from new Residents to be admitted

• Recording health details of Residents; including vitals & temperature

• Administering medications, wound care, clinical documentation, resident assessments and care coordination.

• Provide clinical direction and support to Certified Nursing Assistants while coordinating resident care during assigned shift.

• Monitors Residents’ food and liquid intake and output

REQUIREMENTS:

• Must be able to work as a team member

• Valid NY State LPN license
