Work Environment

This full-time position is onsite and involves providing home care services. You will be visiting patients in their homes, allowing for personalized treatment plans that cater to their unique living situations. Key Responsibilities

• Conduct comprehensive evaluations of new patients, including thorough assessments of their medical history, and functional capabilities to formulate individualized treatment plans.
• Communicate evaluation findings and create tailored therapy goals in collaboration with patients and their families, ensuring their active participation in the rehabilitation process.
• Implement evidence-based treatment protocols that align with the latest health care practices and adapt them based on ongoing patient assessments and feedback.
• Monitor and document changes in patient conditions, recognizing when there is a need for revisions to the treatment plan or further medical consultations.
• Participate actively in discharge planning and follow-up care, ensuring a seamless transition for patients out of therapy with appropriate resources and ongoing support.
• Maintain detailed and timely documentation of patient care activities including initial evaluations, daily treatment notes, progress reports, and discharge summaries to comply with regulatory standards.
• Engage in effective time management practices to meet productivity benchmarks while supporting the team in community outreach initiatives.
• Eagerly pursue continuing education opportunities to enhance your professional development, including mentoring students and sharing knowledge with colleagues.

Patient Population / Client Focus

As a Physical Therapist at Flourish Life, you will work primarily with diverse patient populations, including elderly individuals recovering from surgery or managing chronic conditions, as well as younger patients in need of rehabilitation following sports injuries. The role requires an empathetic approach, understanding patients' emotional and physical challenges while striving to empower them through tailored therapeutic interventions.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution is required. A Doctorate (DPT) is preferred, but not mandatory.
• A valid PT license in the State of Florida is essential for practice.
• Current certification in CPR (BLS) from the American Heart Association.
• A minimum of 2 years of relevant experience is required for this position.

Preferred Experience

• Experience in home health physical therapy, including assessments and treatment planning for in-home services.
• Familiarity with electronic medical records (EMR) systems and telehealth platforms, to enhance patient documentation and care.
• Demonstrated ability to work effectively with a multidisciplinary team.

Compensation and Benefits
