Hospice Nurse Navigator — Admissions & Care Coordinator

Lower Cape Fear LifeCare

Lower Cape Fear LifeCare is looking for a Nurse Navigator to join their dedicated team in Elizabethtown, NC. In this role, you'll be the initial clinical touchpoint for patients and families entering hospice services, managing assessments and care coordination. The ideal candidate has at least 2 years of nursing experience, strong communication skills, and a current RN license in North Carolina.
