Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience depending on the specialty. License: Must have a current license. Certifications: CPR, BLS Must-Have: - Proficient in HomeCare HomeBase (HCHB) and OASIS. - Must have a vehicle, driver's license, and auto insurance. - Must be willing to travel up to 75 miles from assigned care center. Description: Responsible for coordinating and directing the provision of home nursing care according to physician's orders. Care is based on agency policies and procedures through the competent application of the nursing process. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Home visits and periodic office meetings are required. Must have reliable transportation and current liability insurance. Average number of patients per day is 5-8. Each visit and activity holds a different point value, with a goal of 30 points per week. Types of cases include Start of Care, Wound Care, and Follow-up Visits. First-time travelers are not accepted; recent specialty experience is required. Weekend requirements vary per location and will be discussed during interviews. On-call requirements vary; discussion during interviews is encouraged. Mileage reimbursement is provided at the standard IRS rate. Clinicians must accept all patients on their schedule. Documentation must be completed within 24 hours of the appointment. Failure to meet documentation standards may result in cancellation. There are no guaranteed hours associated with this contract. Weekly productivity points of 30 must be achieved, equating to approximately 40 hours per week. All overtime must be pre-approved. Opportunities to float to nearby clinics may be available if patient census is low.
