Case Manager

Coordinate interdisciplinary care for a designated patient population Review inpatient admission records to ensure proper utilization of hospital resources Determine admission for the appropriate level of care Assess and identify discharge needs Coordinate appropriate discharge plans Collaborate with the interdisciplinary team to achieve desired financial and quality outcomes Maintain and enhance payor relationships Requirements Licensure in the state where the hospital resides is preferred Professional disciplines may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker Current BLS certification required Three (3) to five (5) years of inpatient experience, preferably in an acute, IRF, or LTACH setting Core Competencies Demonstrates expertise in coordinating interdisciplinary care and managing patient discharge plans while ensuring optimal utilization of hospital resources. Strong background in maintaining payor relationships and achieving financial and quality outcomes in healthcare settings.
