PERSONAL CARE AIDES for Private Duty care in a Facility or Home Health

Personal-Touch Home Care & Hospice, a leader in the home care industry began operations in 1974 and has grown into a national company providing in home nursing care, IV therapy, wound care, physical therapy, occupational therapy, speech therapy, hospice care, medical social work and personal care services. We are dedicated to the provision of individualized, comprehensive quality patient and family centered care, and believe that each person is a unique individual.

We are seeking qualified Personal Care Aides for our Northside (Peninsula areas) and Southside service locations for all hours and shifts. Cases available for Private Duty in a Facility or Home Health. Shift Hours will vary based on patient need.

REQUIRED QUALIFICATIONS:

· Successful completion of an approved training program in home health services to become a Personal Care Aide.

· Has empathy for the needs of the ill, frail and impaired.

· Is able to communicate information concerning a patient’s condition clearly.

· Displays initiative to work with minimal direct supervision.

· Must be able to read, write and communicate in English.

· Must have reliable transportation and a smartphone.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES:

· Personal care including:

a. Baths—bed, tub, shower 

b. Back rubs

c. Oral hygiene

d. Changing bed linen

e. Assisting the patient with dressing and undressing. 

f. Assisting with grooming—care of hair, including shampoo, shaving, the ordinary care of nails and skin care.

g. Assisting the patient with toileting activities.

h. Assisting in feeding the patient, if necessary

i. Keeping patient’s living area clean and orderly.

· Prepares and serve simple diets.

· Assists the patient in moving from bed to chair, to wheelchair and in walking.

· Reports changes in the patient’s needs and condition to the supervising nurse.

· Keeps simple records.

· Performs incidental household tasks essential to the patient’s health needs, such as:

a. Light housekeeping

b. Making and changing beds.

c. Dusting, vacuuming and mopping the rooms used by patient.

d. Tidying the kitchen, bedroom and bathroom.

e. Shopping for patient if no other arrangement is possible.

f. Personal laundry of patient if no family member available or able

· Performs range of motion and other simple procedures as an extension of therapy services as ordered (with appropriate completed/demonstrated skills/competency).

· Records patient care activities daily.

· Participates in case conferences.

· Prioritizes work/care tasks to ensure all duties and patient visits are completed when assigned.

· Adheres to the Organization’s documentation and care procedures and standards of personal and professional conduct.

· Attends minimum of twelve (12) hours of in-service annually.

· Participates in meetings as requested by the Hospice Director of Patient Services.

· Performs other duties as assigned.

· Note: PCA's are never allowed to administer medications

Standard Shift: ALL hours. ALL shifts to include weekends.

EMR systems: 1 year (Preferred)

License/Certification:

PCA (Required)

Pay: $15.00 to $18.00 per hour
