VibrantCare Rehab is seeking a dedicated  Physical Therapist (PT) to join our team in Sacramento, CA. The Physical Therapist will provide high-quality, patient-centered care while collaborating with patients, caregivers, physicians, and facility staff.

Key Responsibilities

• Evaluate and treat patients requiring skilled physical therapy services.
• Develop individualized treatment plans and monitor patient progress.
• Document evaluations, treatments, goals, progress, and medical necessity accurately and on time.
• Supervise and direct Physical Therapist Assistants and aides as appropriate.
• Communicate effectively with patients, families, physicians, coworkers, and facility staff.
• Participate in facility meetings, case management, and clinical in-services.
• Maintain compliance with state regulations, Medicare requirements, OSHA standards, and company policies.
• Maintain required continuing education and current CPR certification.
• Maintain positive relationships with facilities, clients, and healthcare partners.
• Meet productivity and quality-of-care expectations.

Qualifications

• Graduate of a  CAPTE-accredited Physical Therapy program or substantially equivalent international education.
• Current California Physical Therapist license/registration as required.
• Current  CPR certification required.
• Current  TB test required and maintained according to state guidelines.
• Strong clinical problem-solving and communication skills.
• Excellent organizational, time-management, and planning abilities.
• Ability to work effectively as part of a team.
• Leadership skills to supervise and support PT assistants and aides.
• Strong computer and clinical documentation skills.

Physical Requirements

• Ability to frequently lift and assist patients using appropriate biomechanical techniques.
• Ability to lift up to 50 lbs. independently .
• Ability to walk, bend, stand, and reach throughout the workday.
• Ability to push wheeled equipment.
• Sufficient visual, hearing, and fine motor abilities to perform clinical and documentation duties.

Compensation & Benefits

• Base Salary: $80,000–$90,000/year
