Job Description
- Participates in provider on-call rotation as required.
- Minimum of 1 year experience working with the frail or elderly population is required.
- Not accepting visa candidates
- Provider must be interested in Perm (not accepting Locums)
- Board Certified in Internal Medicine or Family Medicine - REQUIRED
- Board Certified in Geriatrics (Fellowship-trained) or Certificate of Added Qualifications in Geriatric Medicine (to be completed prior to start) - HIGHLY PREFERRED
- IN license (or willing to obtain) - REQUIRED
- Clean Malpractice - HIGHLY PREFERRED
- CDS Certification (by time of start) - REQUIRED
- DEA Verbal Confirmation (by time of start) - REQUIRED
- BLS (by time of start) - REQUIRED
- Minimum of 1 year experience working with the frail or elderly population (add to "Experience" field) - REQUIRED
- Previous supervisory experience (add to "Experience" field) - REQUIRED
- Does the provider require visa support - J1 or H1B? (note in "Additional Submission Details" field) - REQUIRED
- BLS
- Board Certified
- Board Eligible
- Controlled Substance (CSR/CDS)
- DEA
- Indiana
- Previous employment with supervisory experience
