Key Responsibilities:
- Provide care for adult and pediatric patients in New York City through home visits, monitoring, and treatment.
- Conduct assessments and treatment under APP supervision for patients with limited access to healthcare.
- Maintain accurate patient records using HealthPoint and Athena EMR systems.
- Practice with telemedicine support and ensure compliance with clinical standards.
- Act as a conduit for medical treatment in Health Plan Partnerships.
- Engage in health management, medication, and care goal discussions.
- Provide supervised treatment for common diagnoses.
- Participate in ongoing education and adhere to safety protocols.
- Deliver compassionate, patient-centered care.
Qualifications:
- Minimum of two years of experience as a Licensed Practical Nurse (LPN)
- Active and current American Heart Association BLS certification required
- Experience in Emergency Department, Urgent Care, Internal Medicine, Critical Care, Family Practice, or other relevant areas preferred
- Active and current driver's license for 2+ years required
- Current unrestricted NY State LPN license required
- Clinical work experience with pediatrics required
- Phlebotomy experience required
- Fluent in Spanish preferred
- Ability to lift 50-60 lbs
