Clinical Coordinator, RN- Access Management, Full-Time, Mullica Hill

MAJOR FUNCTION:

The Clinical Coordinator RN will operate in direct partnership with IMG practices. To assist our patients in navigating the various complexities of our health system and act as a resource to coordinate the patients' needs with our organization resources. This role will act as a clinical support for the IMG practices by support their pool's and provide clinical triage. This role will complete triage transfers by utilize protocols and clinical judgement to assess and prioritize a patients need and determine the appropriate course of action.

QUALIFICATIONS:

Education & Experience: Associate degree required. Bachelor’s degree preferred. Experience in acute inpatient, rehabilitation, sub-acute, skilled facility, homecare. Experience in a care management role preferred. Experience in Ambulatory care preferred. 3-5 years of direct patient care experience required. Certification/Licensure: Active New Jersey Registered Nurse License is required. Active Basic Life Support (BLS) certification is required.
