In this role, you'll coordinate patient care fromadmission through discharge, ensuring quality outcomes, seamless transitions,and an exceptional patient experience. The Case Manager collaborates closely withphysicians, nurses, and care teams to ensure appropriate level of care,efficient throughput, and safe patient transitions. Schedule: Full-Time | Days | Nights What You’ll Do Responsibilities Assess patient needs and developindividualized care plans Conduct utilization reviews toensure medical necessity and appropriate level-of-care decisions Coordinate admissions, transfers, discharge planning, and community resource referrals Collaborate with physicians,nurses, and interdisciplinary teams to ensure seamless care Identify and resolve barriers tocare transitions and patient flow Educate patients and families ontreatment plans and follow-up care Monitor patient outcomes andidentify opportunities for improvement Ensure compliance with hospitalpolicies, regulatory standards, documentation and payer requirements Advocate for patients throughouttheir healthcare journey Qualifications Graduate of an accredited PracticalNursing (LPN) program BLS certification required, ACLSpreferred Minimum 1-2 years of LPNexperience, preferably in an ER/acute care or Med-Surg setting Strong clinical judgement,communication, critical-thinking, and organizational skills Knowledge of care coordination,utilization review, and discharge planning Excellent teamwork abilities andable to thrive in a fast-paced high-pressure environment Why Join This Opportunity?
