In this role, you'll coordinate patient care fromadmission through discharge, ensuring quality outcomes, seamless transitions,and an exceptional patient experience. The Case Manager collaborates closely withphysicians, nurses, and care teams to ensure appropriate level of care,efficient throughput, and safe patient transitions.
