Care CoordinatorThe Intensive Care Coordinator connects clients with appropriate care resources, collaborates with the therapeutic team to support coordinated service delivery, and promotes healthy decision-making to help clients achieve treatment goals and sustain recovery.What you will be doing:Provide outpatient case management services to the seriously mentally ill population.Provide direct services to severely mentally ill clients to address needs which are unmet as well as information about continuum of careProvide follow-up progress as per need of client to treatment team membersEvaluate client's residential stability and assist those clients who are in need of more stable environmentProvide Basic Living Skills training when necessary as it relates to clients immediate needs while preparing client for less intensive level of care within the communityAssist clients with obtaining medicationMonitor medication compliance, maintain awareness of common side effects, and ensure client is taking medication as directed on a regular basisProvide traditional case management services as well as collateral services such as transportation to appointments, referral agencies, etc.Maintain high quality client documentation and records management utilization electronic charting.Monitor the care and services clients receive in residential settingDevelop and implement Individual Service Plan which consist of goals, resources, and interventions;Review client's progress every three months and revise Individual Service Plan as neededRevise client's goals once met, or progress as they occurAssist mentally ill clients through crisis situations and/or arranging for the provision of such assistance by other professional or personal caregiversPerform incidental care to families of the mentally ill and assist those family members in understanding the mental illnessOther duties assigned.This job description is only a summary of the typical functions of the job and is not designed to be an exhaustive or comprehensive list of all possible duties, tasks, or responsibilities that are required of the employee as they may change, or new ones may be assigned at any time with or without notice.QualificationsWhat we are looking for:Bachelor's Degree in social sciences fieldAcceptable 3 year MVR with valid driver licenseExcellent verbal and written communication skillsState of Alabama Adult Case Management TrainingKnowledge of Community Lay-out (Cullman County Area)Knowledge of Community Agencies and ResourcesBenefitsWhat we offer:Competitive medical, dental, and vision premiumsState Retirement participation through RSA plus an optional 457b plan with a company matchNine (9) paid holidays throughout the yearAbility to continually accrue up to 15 days of PTO a year (unused rolls over)Company-paid Group Life and AD&D insurance and Long-Term DisabilityLicensure reimbursementTuition discounts through learning partnerships with Athens State University and Capella University
