Home Health Registered Nurse (RN) - Full Time

Central Vermont

This is a 13-week contract position.

Days: Monday - Friday (with weekend rotation approximately every 4th week)

Join our community-focused, not-for-profit organization dedicated to enhancing the well-being of central Vermonters. We provide exceptional home health and hospice care, ensuring access to high-quality services for individuals of all backgrounds.

This position is responsible for providing individualized goal-oriented nursing care using the nursing process in accordance with a physician's order. In addition, this position serves as Case Manager in cases where this position is the lead discipline.

Responsibilities:

Deliver individualized nursing care in accordance with physician orders.
Perform case management duties as the lead discipline when applicable.
Utilize the nursing process to develop and implement patient care plans.
Ensure holistic care that promotes health and well-being.

Required Experience / Certifications / Licensure:

Current RN license in the state of practice.
Experience in home health or hospice preferred.
Strong communication and interpersonal skills.
