TRAVEL - RN - Case Manager

RN-Registered Nurse – Case Management (Travel) Onsite contract starts 8/24/2026 for 13 weeks. Day shift 8:00 AM–4:30 PM, 5 days per week. Some weekends may be required. Location HCA HealthONE Swedish, 501 E Hampden Ave, Englewood, CO 80113. Unit Details 200‑bed facility (about 20 beds per unit) Team includes case managers and social workers Patient ratios: SNF 1:40, RN 1:20 Equipment: mobile, computer EMR: Meditech, Midas, Navihealth Responsibilities Conduct discharge planning assessments Perform suicide risk assessments and SBIRT/substance assessments Engage in behavioral health evaluation with suicide risk and substance use assessments Participate in patient and family education regarding palliative care and goals‑of‑care conversations Collaborate with multidisciplinary team to coordinate care transitions Qualifications Minimum 1‑year current acute‑care case management RN experience BLS certification Preferred: ED experience, palliative care knowledge, experience in goals‑of‑care conversations Strong communication skills; professional, dedicated, friendly Adaptable to different units Benefits Competitive compensation and comprehensive benefits, including medical, dental, vision, and retirement options.
