Key Responsibilities\n

• Conduct comprehensive assessments and evaluations of clients' needs using evidence-based practices and culturally sensitive approaches.

• Develop and implement individualized care plans that address the specific mental health challenges faced by clients, emphasizing empowerment and choice.

• Provide direct, focused counseling and therapeutic services to clients, both in individual and group settings, working to establish trust and rapport.

• Facilitate case management services by coordinating care with external providers, family members, and community resources to ensure comprehensive support systems are in place.

• Participate in multi-disciplinary team meetings and case consultations to enhance collaborative approaches to treatment and share insights to improve service delivery.

• Maintain accurate and confidential client documentation in accordance with ethical guidelines and legal standards, ensuring information integrity and accessibility.

• Identify and address unsafe situations through proper reporting and intervention protocols, maintaining a safe environment for clients and staff.

Patient Population / Client Focus\n

In this role, you will primarily serve clients who are dealing with major mental illnesses such as schizophrenia, bipolar disorder, and severe anxiety, along with individuals facing emotional distress due to chronic illnesses or life-limiting conditions within a hospice environment. The focus is on understanding the unique challenges faced by these populations and providing nurturing support to enhance their quality of life during a critical phase.

Qualifications\n

• Bachelor’s degree in Social Work or a related field is required; a Master's degree or equivalent professional experience is preferred.

• Current LCSW licensure in New Jersey is required.

• Proven experience working with diverse populations and low-income communities is highly desirable.

• Strong understanding of mental illness, clinical assessment, and evidence-based practices relevant to hospice care.

• Excellent communication skills, both verbal and written, with the ability to engage clients empathetically.

• Ability to operate effectively in a remote work environment, utilizing technology for telehealth services.

Preferred Experience\n

Experience in hospice, palliative care, or other similar settings is preferred. Familiarity with trauma-informed care practices and crisis intervention techniques is also advantageous for addressing the unique needs of clients in challenging situations.

Compensation and Benefits\n

This full-time position offers an attractive salary range of USD 70,000 to 85,000 annually, commensurate with experience.
