What You'll Do Conduct joint supervisory visits with field nurses. Conduct in-home nursing visits for patients with complex medical needs, providing clinical assessments, treatment, and patient education. Mentor, coach, and develop nursing and paraprofessional staff. Audit clinical documentation for quality and regulatory compliance. Ensure evidence-based nursing standards are followed. Monitor care delivery and identify improvement opportunities. Collaborate with physicians, care coordinators, and interdisciplinary teams. Provide wound care, medication administration, assessments, and patient education. Support Diagnostic & Treatment Center nursing operations as needed. What We're Looking For Active New York State RN license BSN from an accredited nursing program NYSED Infection Control Certification Minimum 3 years of nursing experience Strong assessment, communication, and leadership skills Valid NY Driver's License and reliable vehicle Home Care, PACE, MLTC, Managed Care, or supervisory experience preferred Why Join Matter of Care?
