Department Profile The Children’s Health Center and Lenox Baker Children’s Hospital at Duke University Hospital is comprised of pediatric outpatient clinics. Our team is active in delivering exemplary care for each patient to ensure an excellent patient experience, every time. The Metabolic and Genetics Team at Duke Children’s is active in delivering exemplary care for each patient to ensure an excellent patient experience. Role Overview This position, of Genetics Nurse Navigator, will provide triage support and care coordination for this patient population. It will work closely with the faculty directors of Pediatric Metabolic and Genetics. We are looking for someone with the ability to establish a strong rapport with patients and families by phone, coordinate services, and provide education and nursing support to patients and families in and out of the clinic setting. The work environment is a combination of independent work alongside team collaboration and is supportive, dynamic, and challenging. Ideal Candidate A solid background in Pediatric Nursing Experience with telephone triage or managed care coordination Strong oral and written communication skills; software navigation abilities (EPIC experience preferred) Good organizational skills Flexible attitude with the ability to function both independently and as a team member, assisting patients and families through home care coordination and in the clinic setting Ability to communicate clearly with patients, families, visitors, healthcare team, physicians, administrators, leadership and others Responsibilities RN Care Coordinator will attend clinic as needed to provide clinic coordination. Patient care coordination will be done in an office setting on non-clinic days. Patient population includes patients with various genetic disorders—Down Syndrome, 22q syndrome, glycogen storage diseases, lysosomal storage diseases, metabolic/mitochondrial disorders. Triage calls resolving problems within scope; gather relevant information and coordinate follow-up with providers to develop a plan and convey the plan back to patients. Document communication in Maestro and manage inbox messages regarding care. Patient care coordination involves infusion management, assisting with refills, entering labs/studies needed during appointments, triage patient phone calls, coordinating various appointments and studies, etc. Coordinate new patient appointments, ensuring all necessary medical records, neuroimaging, and labs have been obtained from outside facilities. Coordinate diagnostic testing and outpatient appointments here at Duke or at outside facilities. Work Hours & Shifts Full Time: Hours may vary but will typically fall within a standard Monday‑Friday, 8am‑5pm schedule. Education BSN Required Experience 3 years of nursing experience required. Licensure & Certification Current or compact RN licensure in the state of North Carolina required. Knowledge, Skills, and Abilities Knowledge of scope for a registered nurse; knowledge of and appropriate application of the nursing process. Ability to independently seek out resources and work collaboratively. Ability to establish and maintain effective working relationships. Ability to communicate clearly with patients, families, visitors, healthcare team, physicians, administrators, leadership and others. Ability to teach patients and families in accordance with the nursing plan of care.
