Our mission is simple – to improve the health and well-being of our aging population.Position DetailsLocation: Ashland City, TNSchedule: Monday–Friday, part-time (½ days)Role OverviewAs a Nurse Practitioner with Ultra Health Providers, you will serve as a trusted clinician for residents in your assigned facility. You'll manage acute and chronic conditions, provide preventive care, and partner closely with nursing staff and administrators to ensure exceptional patient outcomes.ResponsibilitiesDeliver primary, urgent, and preventive care to residents in skilled nursing and assisted living settingsConduct admission assessments, physical exams, medication reviews, and depression screeningsCollaborate with attending physicians, nursing staff, and facility leaders to coordinate careCounsel patients and family members on treatment options and changes in health statusManage clinical documentation accurately and in compliance with facility and state standardsQualificationsActive and unrestricted Nurse Practitioner license in TennesseeCurrent national board certification (AANP or ANCC)Minimum 1–2 years of experience in primary care, geriatric, or long-term care settings preferredStrong clinical judgment and communication skillsCommitment to compassionate, patient-centered care$200 - $300 a dayMake a difference where it matters most.
