The Patient Navigator focuses on reducing barriers to care, improving care coordination, addressing social determinants of health, and supporting positive patient outcomes.The ideal candidate is empathetic, resourceful, and passionate about patient advocacy and oncology support services.Schedule HighlightsFull-time position (40 hours/week)Primarily weekday clinic scheduleNo weekends or holidays anticipated based on standard clinic operationsKey ResponsibilitiesPatient Navigation & SupportGuide oncology patients and families through the healthcare system and treatment processProvide emotional support and compassionate patient advocacyHelp patients understand treatment plans, appointments, procedures, medications, and follow-up careReinforce provider education and assist with patient understanding of care recommendationsCare CoordinationCoordinate appointments, referrals, imaging, biopsies, surgeries, and specialty care servicesManage incoming oncology referrals and facilitate timely patient access to careCollaborate closely with providers, infusion staff, schedulers, billing, pre-certification, registration, laboratory, and other departmentsAssist patients in establishing primary care and specialty care services when neededBarrier Assessment & Resource NavigationAssess barriers to care including:TransportationFinancial concernsInsurance issuesSocial determinants of healthCommunity support needsConnect patients with:Community resourcesCancer support organizationsFinancial assistance programsEducational resourcesAssist with grants, financial assistance applications, and resource referralsPatient EducationProvide education and supportive resources related to:Cancer diagnosesTreatment plansMedicationsWellness and supportive careTailor education to patient learning needs and cultural considerationsInclude family/support persons in education when appropriateDocumentation & Program SupportDocument patient interactions, barriers, referrals, and coordination activities in the electronic health recordMaintain accurate patient tracking and navigation recordsMonitor Artera messages and patient communication platformsSupport quality improvement initiatives and department KPIsCollaboration & CommunicationServe as a liaison between patients, providers, families, and multidisciplinary care teamsParticipate in care coordination and team communication activitiesMaintain confidentiality and HIPAA compliance at all timesEducationHigh school diploma or equivalent requiredAssociate’s or bachelor’s degree in Health Sciences, Public Health, Social Work, Psychology, Healthcare Administration, or related field preferred but not requiredExperienceMinimum two (2) years of experience in healthcare, patient services, case management, community health, or related field requiredExperience working with oncology patients, chronic disease populations, patient navigation, and community health outreach strongly preferredCertificationsCurrent AHA BLS (Heartsaver) certification requiredSkillsStrong communication and interpersonal skillsAbility to work effectively with diverse patient populationsStrong problem-solving and resource navigation abilitiesKnowledge of:Community resourcesFinancial assistance programsHealthcare systemsPatient education techniquesProficiency with:Electronic health recordsMicrosoft OfficeEmail and spreadsheetsOncology management softwareAbility to exercise initiative, judgment, and independent decision-makingWork EnvironmentThis position works primarily in an outpatient oncology clinic setting with frequent communication with patients, families, providers, staff, and community organizations. The role may involve exposure to blood, bodily fluids, communicable disease, chemotherapeutic agents, hazardous materials, and emotionally challenging situations.Compensation & BenefitsBase Compensation Range: $19.50 - $30.14 hourly, based on 0-22 years of relevant experience.
