We CARE for our patients like they are our own FAMILY.Beaumont Home HealthSchedule: Three days weekly, 8:15AM - 5:00PMWhat We Are Seeking:The Licensed Practical Nurse (LPN) I provides comprehensive skilled care as ordered by the attending physician by following the order sets created by the RNCM. The LPN I educates patients and their family members and ensures the safety of the patient. The LPN I is accountable for completing accurate documentation and remains compliant with all legal rules and regulations. The LPN I is responsible for all practices and duties within the scope of practice as outlined by the state.Key ResponsibilitiesProvides skilled nursing care in a patient's home as ordered by the attending physicianPerforms care by following order sets and goals made by RNObserves and monitors patient conditionsNotifies RNCM of any changes in patient conditionAdministers medication as prescribed by the physicianCounsels patient and family on the disease/injury processes and how to manageIncorporates patient and family in development of plan of careHelps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisionsListens to patient and family membersCompletes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelinesDocuments all aspects of treatment, assessments, and patient educationQualificationsMinimum of one year of experience as a LPN in an acute care setting is required; two years is preferred. Home care experience is desired.Ability to effectively communicate and create positive impressions with patients, families, physicians and co-workers.Ability to remain calm, have patience and be accommodating. Compassionate and caring while working with patients.Knowledgeable on nursing best practices.Ability to make appropriate nursing judgments.Ability to identify a situation and handle it with the best possible solution.Detail-oriented and observant.Disciplined style of work ethic with the ability to prioritize and be timelyAbility to follow directions and work as a team member.Valid driver's license and auto insurance with your name as a listed driver.Education and CredentialsLicensed Practical Nurse with current license in the state of employmentWe'll help you put your passion for patient care to work.
