On-Site Primary Care NP - Advanced Practice Provider

CVS Health Corporation

Oak Street Health is recruiting a Nurse Practitioner to deliver value-based primary care to adults on Medicare in its Providence clinics. The NP will work on a collaborative care team to provide longitudinal care, conduct wellness visits, and educate patients on management plans. Responsibilities include in-office visits, care coordination, population health leadership, and providing home visits as needed. Strong NP certification and active licensure are required, with US work authorization and #J-18808-Ljbffr CVS Health Corporation
