Physical Therapist (PT)
Location: Sullivan, IN
Job Type: Full-Time
Setting: Physical Therapy
Key Responsibilities
Evaluate patients and develop individualized treatment plans.
Provide skilled physical therapy treatments.
Monitor patient progress and communicate with providers.
Supervise and collaborate with PTAs and other healthcare professionals.
Maintain accurate and timely documentation.
Participate in quality improvement initiatives.
Manage approximately 8-9 patient visits per day .
Utilize Meditech EMR .
Qualifications
B.S., M.S., D.P.T., or equivalent degree in Physical Therapy.
Active Indiana PT license.
Current CPR certification.
Minimum 1 year of PT experience required.
Strong clinical, communication, and interpersonal skills.
No specific specialty experience required.
Schedule
Full-Time
9:00 AM-5:30 PM, 4 days/week
Weekend coverage approximately 1 weekend every 8-9 weeks
10% differential for night shifts when applicable.
