Registered Nurse – Home Health

Schedule: Friday, Saturday, Sunday and Monday 8 am to 6 pm.

Assignment Details: Some call may be scheduled, but only on an emergency basis.

Requirements

• Must have OASIS (Outcome and Assessment Information Set) experience.
• Home Health and Hospice experience required.
• Experience traveling from patient to patient for home visits.
• Must be able to manage and adjust to changes in schedule.
• Must have a driver's license in good standing.
• BLS required.
• EMR system used is HealthWyse (Careficient).

Responsibilities

• Provide comprehensive nursing care in a home health setting.
• Conduct patient assessments and develop care plans based on OASIS standards.
• Collaborate with interdisciplinary teams to ensure optimal patient outcomes.
• Educate patients and families on care plans and health management.
• Monitor and document patient progress and adjust care plans as necessary.
• Adhere to all regulatory and compliance standards.
