You’ll collaborate closely with physicians, nursing, and therapy teams to deliver individualized, patient-centered care.Key ResponsibilitiesPerform evaluations within 24 hours (or 1 business day) of physician referralDevelop and implement individualized treatment plans with physician approvalProvide therapy services in accordance with treatment plansMonitor patient progress and adjust care as neededMaintain accurate clinical documentation (evaluations, progress notes, discharge planning)Participate in care plan meetings, utilization review, and interdisciplinary conferencesEducate residents, families, and staff on therapy programs and techniquesSupervise SLPAs and Clinical Fellows (CFY) as applicableConduct staff training and in-service educationEnsure compliance with all regulatory and facility standardsQualificationsMaster’s or Doctoral degree in Speech-Language PathologyActive, unencumbered SLP license in South Carolina (required)CCC-SLP preferredExperience in a skilled nursing or rehab setting preferredKnowledge of speech, language, cognitive, and swallowing disordersExperience with PointClickCare (PCC) and Casamba preferredStrong clinical, communication, and documentation skillsAbility to work both independently and as part of a teamWhy Join Chandler Creek Post Acute?At Chandler Creek, you’ll join a compassionate and collaborative team where your expertise is valued and your growth is supported.
