Our services include Personal Care, Companion Care, Skilled Nursing, Private Duty Nursing, and the Georgia Pediatric Program (GAPP). We are seeking experienced Registered Nurses to complete client assessments, supervisory visits, and clinical oversight throughout Georgia.

Position Summary

The Registered Nurse (RN) Contractor is responsible for conducting comprehensive client assessments, completing supervisory visits, developing and updating care plans, and ensuring the delivery of safe, high-quality home care services. The RN works collaboratively with physicians, caregivers, agency staff, and families to ensure compliance with agency policies, Medicaid regulations, and applicable state and federal standards.

Essential Duties and Responsibilities

Clinical Assessments

• Conduct initial in-home nursing assessments for new clients.
• Complete reassessments as required by payer or agency policy.
• Evaluate medical, functional, psychosocial, and environmental needs.
• Determine eligibility for services and recommend the appropriate level of care.
• Identify safety risks and provide recommendations.

Care Planning

• Develop individualized plans of care based on assessment findings.
• Update care plans when a client's condition changes.
• Review physician orders and ensure implementation.
• Coordinate care with physicians and other healthcare professionals.

Supervisory Visits

• Conduct routine supervisory visits for caregivers providing:
• • Personal Care Services
  • Companion Care
  • Skilled Nursing
  • GAPP services
• Observe caregiver performance and ensure competency.
• Evaluate compliance with care plans and agency standards.
• Provide coaching, education, and corrective guidance as needed.
• Document findings and recommend additional training when necessary.

Clinical Oversight

• Monitor client progress and changes in condition.
• Communicate significant findings to physicians and agency leadership.
• Participate in interdisciplinary case conferences.
• Assist with hospital discharge planning and continuity of care.

Documentation

• Complete accurate and timely clinical documentation.
• Submit all required documentation within 24 hours of each visit unless otherwise directed.
• Maintain records in accordance with HIPAA, Medicaid, CHAP, and agency requirements.

Quality Assurance

• Participate in quality improvement activities.
• Assist with state surveys, payer audits, and accreditation reviews.
• Ensure compliance with Georgia Medicaid and agency policies.

Qualifications

Required

• Active Georgia Registered Nurse (RN) license in good standing.
• Minimum of two years of nursing experience.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Ability to travel throughout assigned service areas.
• Ability to pass a criminal background check.

Preferred

• Home Health experience.
• Private Home Care experience.
• GAPP experience.
• Pediatric nursing experience.

Knowledge and Skills

• Comprehensive nursing assessment.
• Care planning and clinical documentation.
• Supervisory and leadership skills.
• Strong communication and interpersonal abilities.
• Knowledge of infection prevention and control.
• Familiarity with electronic medical records (EMR).
• Understanding of Georgia Medicaid regulations and home care standards.

Physical Requirements

• Ability to travel to client homes.
• Ability to stand, walk, bend, kneel, and lift up to 25 pounds.
• Ability to work in various home environments.

Compensation

This is a 1099 Independent Contractor position.

Compensation may include:

• Flat rate per initial assessment.
• Flat rate per supervisory visit.
• Hourly rate for clinical consultation or additional nursing services.
• Mileage reimbursement when authorized by the agency.
