**Job Description:**

We are seeking a dedicated and compassionate Community Health Worker to join our team. As a vital member of our healthcare organization, you will play an essential role in bridging the gap between healthcare providers and the community to improve health outcomes and quality of life.

**Key Responsibilities:**

- Establish respectful and effective communication with community members to understand their health needs and concerns.
- Facilitate access to healthcare services by providing information, guidance, and support to individuals and families.
- Assist in the development and implementation of health education programs and workshops tailored to community needs.
- Provide culturally competent advocacy to eliminate health disparities and promote health equity.
- Collaborate with healthcare professionals, social workers, and community organizations to coordinate comprehensive care.
- Conduct outreach activities to connect with underserved populations and build trusting relationships.
- Document and report on community health needs and activities to support data-driven decision-making.

Core Duties and Responsibilities  

- Serve as a field-based member of either the ECM and/or Community Supports Care Teams, leveraging lived experience to connect members with the healthcare system. 

- Focus on supporting members who are difficult to engage and those facing cultural or linguistic barriers through field-based contacts and accompaniment to appointments. 

- Assist the Program Managers, Care Managers and Coordinators with panel management by performing delegated activities as assigned. 

- Engage with eligible members in clinics and the community to promote enrollment and participation in services. 

- Help members navigate the healthcare system and community resources, providing follow-up by phone and in-person. 

- Support members in participating in medical and/or behavioral health care by identifying and overcoming barriers, sharing relevant information with the multidisciplinary team and providers. 

- Utilize evidence-based approaches, such as Motivational Interviewing, to engage members and promote collaboration between members and their care teams. 

- Collaborate on member care issues with other CHTC team members, participating in weekly systematic case reviews and consulting with Program and Care Managers on clinical matters. 

- Accompany ECM members to office visits as needed, ensuring accessibility within guidelines. 

- Engage with patients in hospitals through in-person visits when possible and follow up with those recently discharged. 

- Distribute health promotion materials and assist with scheduling appointments with other ECM team members as appropriate. 

- Connect members with necessary social services and supports, advocating on their behalf with healthcare professionals. 

- Monitor treatment adherence, including medication and Shared Care Plan goals. 

**Qualifications:**

- High school diploma or equivalent; additional education in health-related fields is a plus.
- Previous experience in community outreach, public health, or a related field is preferred.
- Strong interpersonal and communication skills.
- Ability to work collaboratively with diverse populations and professionals.
- Knowledge of local community resources and healthcare systems.
- Flexibility to travel within the community as needed.
- Basic proficiency in computer and software applications for documentation and reporting.

- Experience as a community health worker or in a similar role, particularly in diverse communities. 

- Excellent interpersonal and communication skills, with the ability to engage effectively with patients from varied backgrounds. 

- Ability to work collaboratively in a multidisciplinary team environment. 

Preferred   Qualifications:

- Lived experience in such areas such as homelessness, unstable housing, substance use disorders, mental health issues, abuse, family incarceration, exposure to violence, and financial instability. 

- Familiarity with evidence-based engagement strategies, such as Motivational Interviewing. 

- Bilingual proficiency (e.g., Spanish) is a plus. 

- Experience with CalAIM and/or ECM.
