Direct Support Professional - Adult Day Habilitation Location: CP Rochester – 3399 S Winton Rd, Rochester, NY 14623 Salary: $18.00 - $20.00 commensurate with experience Signing Bonus: $300 signing bonus (25% at 3 months, 25% at 6 months, 50% at 1 year) Full‑time 35‑hour week, Monday‑Friday 8am‑4pm. Summary / Objective: Ensure safety and supervision of all individuals focusing on fulfilling the agency’s Mission, Vision and Values and providing services which meet or exceed the goals of the individual, department and the agency. Requirements High School Diploma or GED equivalent required. Experience working with people with developmental disabilities preferred. Valid and clean New York State Driver’s License. Demonstrated ability to effectively communicate orally and in writing. Ability to become CPR and first aid certified and successfully complete required training courses. Essential Functions Imparts agency philosophy of “Equal Opportunity, Independence and Realization of Individual Potential” to children, families, program staff and community resources. Maintains confidentiality of information relating to individuals and their families in accordance with agency policy. Provides oversight to ensure safety and supervision of individuals; may be required to work in the agency’s residential homes during emergencies. Provides assistance, training and supervision with activities of daily living, helping each individual improve their independence while addressing specific individualized outcomes. Accompanies individuals to off‑site locations/activities, helping them access community resources (shopping, recreation, entertainment, etc.). Assists individuals with planning and accessing activities of choice. Provides input regarding individual skill development and functional assessment for team meetings and progress updates. Acts as a role model for program participants by interacting professionally, dressing appropriately, and practicing good personal hygiene. Maintains necessary documentation per regulations and agency policies. Communicates regularly with agency personnel to share information and discuss concerns; provides feedback to supervisor regarding individual and/or program concerns. Assists individuals in working toward achievement of their goals, while ensuring adherence to applicable Support Plans. Manages crisis situations per agency policies and procedures. Supports and encourages individuals to make decisions ranging from basic choices to long‑term goals. Provides emergency and first aid treatment within scope of training until relieved by another staff or medical professional. Attends all departmental or related meetings/trainings; participates in development and maintenance of person‑centered Action Plans. Participates in the team decision‑making process and carries out decisions consistently per agency/department policy. Presents a positive and professional public image; interacts politely with individuals, families, staff and community members. Attends all required training and any other training assigned by supervisor. Participates in conferences, workshops and in‑service training as needed to enhance job skills and personal growth. Physical Demands The employee must regularly talk or hear, sit, stand, walk, use hands to finger, handle, feel, and reach with hands and arms; frequently lift and/or move objects up to 50 pounds. Travel Primarily local during business days; may travel to a residential house in emergency situations.
