What You Need to Know: Location: Chicago, IL 60601 Full-time position (40 hrs/week) | Monday-Friday Salary: $55.00 per hour

1. weekend per month (Saturday & Sunday, day shift with scheduled patients)
2. holidays per year (1 summer / 1 winter)

No on-call required Local travel to residential homes Key Responsibilities: Perform Physical Therapy assessments and develop treatment programs Record and report patients’ reactions to treatment and condition changes Instruct patients and caregivers on therapy programs and device use Supervise Physical Therapy Assistants and, when appropriate, Certified Nursing Assistants Prepare reports and participate in staff and Agency development activities Valid Physical Therapy license Prior experience in home health or a similar setting preferred Commitment to Diversity Harris Fields Search Partners, N.A., and its clients are committed to fostering a diverse and inclusive environment in the employment process.
