Hematology/Oncology Clinical Pharmacist

Role Overview This clinical pharmacist position involves directing and implementing pharmacy services, providing clinical consultation, collaborating across healthcare teams, developing clinical guidelines, participating in committees, and maintaining regulatory compliance to support high‑quality patient care. Responsibilities Direct and initiate development, implementation, and maintenance of clinical pharmacy services. Provide clinical consultation to the Medical Staff and other health care providers, ensuring cost‑effective therapeutic alternatives. Communicate and document all clinical activities, including medication orders, pharmacokinetic dosing, adverse drug reactions, and errors. Foster teamwork and ensure delivery of high‑quality patient care; perform staff duties as needed. Collaborate with all providers for timely resolution of concerns and queries. Deliver ongoing education and drug information to physicians, nursing, pharmacists, and other professionals; develop educational materials and competencies, and provide in‑services as needed. Facilitate development and implementation of treatment guidelines, pathways, protocols, formulary changes, medication‑use evaluations, and drug information reports. Actively participate on clinical committees, including Pharmacy and Therapeutics Committee and others; liaise with healthcare practitioners and maintain relationships with physicians, medical staff, and nursing. Demonstrate awareness of documentation standards, JCAHO, CMS, State Board of Pharmacy requirements, departmental and hospital policies, and system initiatives. Education and Experience Pharm.D. degree with 2 years of inpatient clinical pharmacy experience or completion of a Pharmacy Residency. Licenses and Certifications Current license or registration to practice pharmacy in Louisiana. Board certification in applicable specialty area preferred. Skills and Abilities Adherence to LCMC Health rules and regulations and standards of accreditation bodies. Maintenance of state license and continuing education requirements. Proficiency in ordering within an Electronic Health Record system and basic inventory management knowledge regarding drug shortages. Strong prioritization and self‑motivation skills. Effective communication at all levels, both written and verbal. Willingness to accept additional responsibilities as needed. Work Shift Day shift (United States of America).
