Job Summary Major Duties, include but are not limited to:

• Provide medical services to Veterans on an outpatient basis in the primary care setting. Disease processes include, but are not limited to: diabetes, hypertension, hyperlipidemia, chronic obstructive pulmonary disease, mental health disorders, comprehensive women's health care, etc., with a major focus on preventive medicine.
• Lead the Patient Aligned Care Team (PACT) approach in the delivery of care, which emphasizes a team-based approach to healthcare.
• Document all medical information in an electronic medical record.
• Perform medication reconciliation.
• Provide effective delivery of outpatient care while meeting and exceeding compliance standards.
• Maintain current clinical privileges, licensure, Basic Life Support (BLS), and all other requirements as appropriate.
• Cooperate with, and participate in, medical duties or functions related to quality of care and peer review along with improvement/performance improvement activities at the direction of the Chief of the Medical Service.

Work Schedule: FT, Monday - Friday, 8am to 4:30pm
