Location: East Valley, AZ (Mesa / Apache Junction / Gilbert)
Compensation: $75,000+ / FFS DOE
Status: Full-Time, W2 Employee
Schedule: Monday - Friday, 8 AM - 5 PM
Position Overview
We are seeking an experienced and compassionate Mobile Licensed Behavioral Health Counselor to support a geriatric patient population within a mobile primary care practice . This role partners closely with medical/psychiatric providers to deliver integrated, patient-centered behavioral health services directly in patients' homes, assisted living facilities, and other community settings.
This position is ideal for clinicians who are highly independent, organized, and comfortable working with medically complex older adults. The counselor is responsible for managing their own schedule, providing in-person counseling services, and ensuring all documentation meets Medicare and regulatory compliance standards .
Providers are independent contractors. Compensation is fee-for-service , based on completed visits and submission of compliant documentation.
Required Licensure
- Candidates must hold an active, unrestricted license in one of the following:
- Licensed Professional Counselor ( LPC )
- Licensed Clinical Social Worker ( LCSW )
- Clinical Psychologist (PhD or PsyD)
Key Responsibilities
Geriatric Behavioral Health Care
- Provide individual therapy and supportive counseling to geriatric patients (primarily Medicare population)
- Address common conditions such as depression, anxiety, adjustment disorders, grief, cognitive decline, and chronic illness-related behavioral health concerns
- Conduct comprehensive psychosocial and behavioral health assessments
- Develop and implement individualized, goal-oriented treatment plans
- Utilize evidence-based, age-appropriate therapeutic interventions
