Hospice RN Case Manager
Preferred Home Health Care & Nursing Services
Develop and implement individualized care plans for patients. Monitor patient vitals and administer medications as prescribed. Educate patients' families on care procedures and health management. Document clinical notes accurately and maintain comprehensive records. Collaborate with an interdisciplinary team to ensure patient comfort, dignity, and timely responses throughout the care process.
