Home-Based Nurse Practitioner — Telehealth & In-Home Care

Alumus

Alumus is seeking a Nurse Practitioner to redefine home-based primary care through patient visits at home and via telehealth. The role focuses on chronic, principal, transitional and wellness care for seniors, reducing readmissions and costs. You will operate in a collaborative team with flexible employment models including full-time, part-time or 1099 independent contracts. Expect a manageable caseload of 16-20 patients weekly, with profit share after 90 days and a focus on quality care across #J-18808-Ljbffr Alumus
